To expedite your visit, new patient forms can be completed prior to your arrival. These forms include Patient Information HIPPA and financial information. You may download and print them from this website, fill them out, and bring them with you to your appointment (Use "Patient Forms" tab above). We will need one set for each patient we are seeing.
If you have x-rays from a previous dentist, please have these available to us at your first appointment. Most dental offices request your signature for consent prior to forwarding them. They can be mailed to us or e-mailed to us at taocolvin@gmail.com. Otherwise, we will take the necessary x-rays during your visit with us.
Good preventative care can reduce dental problems and prolong the life of your teeth. At your regular hygiene appointments, you can expect one of our capable hygienists to perform your cleaning and take the appropriate x-rays. Also, an important part of their job is to educate you in the proper care of your teeth. The doctor will perform an oral examination as well as a complete examination of your teeth. For most patients, we recommend dental hygiene appointments every six months and x-rays annually.
To maintain optimal dental health it is important to restore teeth that have been damaged, decayed, or lost. The procedures can include fillings, crowns, and bridges or implants. Once the doctors have examined your teeth and evaluated your dental and medical history, they will provide you with a diagnosis and treatment options.
If you have been advised by your physician or surgeon to take an antibiotic pre-medication prior to a dental appointment due to a medical condition (such as joint replacement, etc.) please make sure that you have the proper medication prior to the day of your dental visit. We prefer to have your physician prescribe the type of antibiotic they would like you to take, as well as the length of time it will be needed. We are happy to call refills in to the pharmacy of your choice.
Our staff takes safety seriously. Infection control is of the utmost importance to our team. Standards recommended by the American Dental Association (ADA), Center for Disease Control (CDC), and Occupational Safety and Health Administration (OSHA) are strictly enforced to maintain sterilization and prevents cross contamination. These standards enable us to protect our patients and our staff.
Dr. Clara Tao grew up in Lincoln, Nebraska. After attending the University of Nebraska, she graduated from the University of Nebraska College of Dentistry in 1981. She has had a passion for practicing dentistry and caring for patients for 35 years. Dr. Tao is a member of the American Dental Association, the Colorado Dental Association and the Colorado Springs Dental Society. She has served as president of the University of Nebraska College of Dentistry Alumni Association Board of Directors and enjoys volunteering for Kids in Need of Dentistry here in Colorado Springs. Her hobbies are arts and crafts, reading, games, crocheting and enjoying family time in the Colorado Springs community.
Dr. Kate grew up in Monument, Colorado and graduated from Lewis-Palmer High School. After attending the University of Kansas, she earned her D.D.S. degree from the University of Nebraska College of Dentistry. Following graduation she practiced in Lincoln, Nebraska until she and her family moved back to join the practice in early 2016. Dr. Kate has been involved in many dental missions and outreaches, and her favorite experience was having the opportunity to practice dentistry in Peru on the Desana Dental Mission. Dr. Kate is a member of the American Dental Association, the Colorado Dental Association, and the Colorado Springs Dental Society. In her spare time she enjoys traveling, watching sports, skiing, hiking and spending time with her husband, Chris, and their three children, Samantha, Jack & Hannah.
Dr. Ben grew up in Maplewood, Minnesota, graduating from Hill- Murray High School. He attended Creighton University for his undergraduate studies and continued on there to receive his D.D.S. degree. Dr. Ben wanted to be a dentist from a young age after seeing his the wonderful way his childhood dentist cared for him and his family. He knows going to the dentist can be a source of stress and will always go out of his way to make his patients feel comfortable and respected. Dr. Ben is a member of the American Dental Association, the Colorado Dental Association.
